How Does PureWick Stay in Place? Setup & Use Guide
Most caregivers hear about the PureWick system and immediately ask the same question: if there is no adhesive, no strap, and nothing holding it on, how does it actually stay put all night? The idea of an external catheter that relies on positioning alone sounds unreliable until you understand the mechanics. Once you do, setup takes a few minutes, and most caregivers find a consistent routine within the first few nights.
This guide covers the positioning mechanics, a step-by-step setup walkthrough, troubleshooting for common problems, and how insurance coverage works for eligible patients.
How PureWick Stays in Place
The PureWick Female External Catheter system stays in place through two forces working together. The body’s natural perineal contours cradle the wick when the patient is lying on her back or in a semi-reclined position. And the pump’s gentle suction creates enough pull to keep the wick seated against the skin without requiring adhesive or straps.
Nightly Setup: Step by Step
What You Need at the Bedside
- PureWick collection unit (suction pump and canister) on a flat stable surface
- One fresh PureWick wick — single-use, never reuse a wick from a previous night
- Tubing and elbow connector
- Optional: skin barrier cream if the patient has sensitive skin
- Optional: mesh underwear to help hold positioning
Setup Steps
- Plug the power cord into the device outlet and into the A/C power outlet. Ensure the power switch is turned off before connecting.
- Place the collection canister in the base and press down firmly on the lid of the device, making sure the lid is sealed.
- Attach the pump tubing to the PureWick Urine Collection System connector port and the connector port on the collection canister lid.
- Attach the elbow connector to the collector tubing and connect the elbow connector to the connector port on the collection canister lid.
- Connect the other end of the collector tubing to a PureWick Female External Catheter.
- Position the patient on her back or in a semi-reclined position. Place the wick externally along the perineal area with the soft absorbent side facing the skin. The tubing should run between the legs toward the collection unit at the bedside.
- Turn on the pump. Confirm the tubing connections are secure and airtight.
Morning Routine
- Turn off the pump
- Gently remove the wick and dispose of it in regular household trash
- Empty the canister and clean it per the manufacturer’s instructions
Troubleshooting Common Positioning Problems
Wick Displacement Overnight
Wick displacement is the most commonly reported issue during the first week. If it occurs, check that the tubing is not pulling taut when the patient moves. Moving the canister closer to the bed can reduce the angle of pull on the tubing. The use of mesh underwear is also recommended to help hold the wick in position. Most caregivers report finding their reliable setup by night three or four.
Side Sleepers
The PureWick system works best for back sleepers and semi-reclined patients. Side sleeping can cause the wick to shift.
Strategies that caregivers have found helpful:
- Use a body pillow or wedge pillow behind the patient to encourage a back or slightly angled sleeping position
- Place a small pillow between the knees to slow rolling
- Use mesh underwear to help hold the wick in position
- Do a repositioning check after the patient falls asleep and again before you go to bed
For patients who are committed side sleepers and cannot be repositioned, talk to your physician about whether external collection is the right fit.
Skin Irritation
Skin irritation is uncommon but possible, particularly for patients with fragile or sensitive skin. Applying a thin layer of barrier cream before placing the wick can prevent friction-related redness. If irritation develops and persists, contact RA Fischer for guidance on compatible skin care products.
Suction Issues
If the patient reports discomfort or suction seems weak, check that all tubing connections are secure and airtight per the product manual. If the issue persists, contact RA Fischer Co. for guidance.
Wick Replacement
PureWick wicks are strictly single-use. Reusing a wick increases infection risk and reduces suction effectiveness. For patients using the system nightly, that is approximately 30 wicks per month. RA Fischer Co. carries both the Original (blue) and Flex (purple) wicks. The Flex wick is more flexible, which some patients find more comfortable depending on anatomy and positioning.
Getting PureWick Through Insurance
For most eligible patients, Medicare Part B covers the PureWick system and monthly wick refills as durable medical equipment. RA Fischer Co. is a Medicare-accredited DME supplier and handles the authorization process on your behalf. For Medicare patients, monthly refill coordination is part of the service. Frequently Asked Questions
-
How does the PureWick external catheter stay in place without adhesive?
It relies on the body’s natural perineal contours to cradle the wick when the patient is lying on her back, and gentle suction from the pump maintains consistent skin contact. Mesh underwear can also be used to help hold positioning. No adhesive or straps are needed.
-
How often should I replace the wick?
Every night. Wicks are single-use. Reusing them compromises hygiene and suction performance.
-
Can PureWick be used by side sleepers?
It is designed for back sleepers and semi-reclined patients. Side sleeping can cause displacement, though pillow arrangements, mesh underwear, and periodic repositioning checks can help for patients who shift during the night.
-
What causes the wick to shift, and how do I fix it?
Tubing tension and body positioning are the most common causes. Check that the tubing is not pulling taut, that the canister is positioned close to the bed, and consider using mesh underwear to help hold the wick in place.
-
Is the PureWick system covered by Medicare?
Yes, for eligible patients. Medicare Part B may cover the pump, canister, and monthly wick supply as durable medical equipment. See our guide to is PureWick covered by Medicare for details.
-
How long does nightly setup take?
Setup takes a few minutes and becomes faster as the routine becomes familiar. Morning cleanup is similarly quick.
-
Where can I order replacement wicks?
RA Fischer Co. carries Original and Flex wicks in 10-packs and 30-packs. For Medicare patients, monthly refill shipments are coordinated through your coverage. Contact us or visit the PureWick insurance page to get started.
